Default Folder View in "Save As" Window in Photoshop CC 2015

i'm on 2013 mac pro el capitan.  installed ps cc 2015.  when go save in menu, give me complete hierarchy in list view, want.  however, if directly try save again, bottom screen shot without hierarchy in list view.  it's random.  closing out ps , restarting makes no difference.  called tech support , guy told me supposed this.  after showed hierarchy him once, second time didn't.  did 1 more time , did same thing....then each subsequent time gave list view without hierarchy.  said it's not ps related.  however, no other program shows behavior.  other programs list view hierarchy every single time.

Maximum number of authorizations reached!
