I am trying to save a interactive web form as an interactive PDF

i trying save interactive web form interactive pdf.

 

the drop down values not carry on when form saved pdf. please assist!

just sure we're on same page (no pun intended), you're saying have webpage not pdf form , want turn in pdf fillable form fields. in essence making duplicate of webpage in pdf form?

 

if so, can't done acrobat reader. acrobat, not believe can done automatically. need print webpage pdf go in , add form fields manually.

 

if need different, please supply more detail.
