Unwanted shift line brush occuring randomly while using the brush and eraser on tablet+mac

i'm using cintiq 22hd on macbook air , whenever i'm using either brush or eraser create line if i've held down shift key 60% of time. don't have buttons on tablet assigned shift-key , shift key on keyboard not stuck can type fine without seeing random capitalizations. it's impossible use, have clue on fix?

please try , reset photoshop preferences default.

 

steps:

to re-create preferences files photoshop, start application while holding down ctrl+alt+shift (windows) or command+option+shift (mac os). then, click yes message, "delete adobe photoshop settings file?"
