Warning when I upload my site

when upload site, have error/warning message shown on screenshot. hosting company has php servers , host thousand of websites, not on end. means? problem might related use of simple contact form. have noticed there no validation of field, entering alphabetic characters in field should contains numeric character (like phone number). form may have built without validation. idea? thanks.

if copy site url entered in ftp upload dialog browser site uploaded muse load? if not, it's not right url.

Maximum number of authorizations reached!
