When saving for Web & Devices, using Photoshop CS5, about 1 in every 60 images does not actually save/rewrite (which is what it should do). I work high volume; what could be causing this?

since work relatively fast-paced , high-volume, can cause lot of snags if edited image not save & rewrite. haven't noticed commonalities in images photoshop chooses not save , problem has persisted months. ideas?

just installed acrobat 11.0.13 on windows 7 sp 1 ultimate 64-bit machine.  when try run, > check updates, error below.  happening on several machines.  in domain group policy not restrict updates.  have tried running acrobat "run administrator" , have tried running update service administrator.  no luck.     common acrobat nonsense, there not policy acrobat !!! just missing unnecessary adobe arm !!! this time when see stupid error message installer, not install updater as  !!! but avoid annoying offer update usefulness acrobat dc (ok adobe give try, respect effort, no more reader (sorry adobe !!!) remove complete adobe arm folder, acrobat not need , loose nothing of functionality !!! however paid acrobat xi pro , not need degradation it  dc !!!
