why are all the rows in layer panel merging into one as content is added in Photoshop cs6?

i have background layer, , open new layer shows on layer panel above background usual, named layer 1. try add image new layer, , background layer disappears, , image thumbnail appears in 1 remaining row, named layer 0. adding further layers has same result, layer 0 remaining visible images added.  clicking on background header @ top of screen results in individual layers appearing, unable work of images.

          new photoshop, , trying follow video tutorial, blend 2 images one, , copying each instruction step step results not comply results in video. have tried add screenshots, reason aren't being accepted.

could please post screenshots pertinent panels visible illustrate issue?

to me sounds not placing images in image @ all, might opening them.

Maximum number of authorizations reached!
