Button Action on WebHelp Toolbar (RH 11)

i'm having trouble figuring out what's wrong buttons in skin.  issue when first opened, of labels on toolbar buttons display.  however, when click on button right thing happens (toc displays, or index displays, etc.) button "disappears".  how can keep name of button visible while perhaps having change color or indicate it's active one?

 

i've attached screen shots hope help.  come through post.

 

any appreciated.

do have same problem other skins? new 1 me, i'm not sure whether it's skin related or general issue.

Maximum number of authorizations reached!
