Create form in word with checkboxes, radio buttons, comment fields & signatures


a couple of years ago created form filled out checkboxes & radio buttons, sent signatures. example below:

is still possible use, or need drag&drop buttons?

you can add form acrobat & prepare pdf form, can add fields & required buttons, further add action & properties.

refer kb doc. acrobat | creating , distributing pdf forms.
