"select location for download" dialog doesn't save on Safari OS X

i'm using latest os x, safari , flash player. dialog "select location download" doesn't allow me save file. instead, sort of freezes safari behaving unresponsive model dialog box.

i've followed how reset safari on mac | macissues but makes no difference. put me on track flash though site i'm able reproduce issue in (pixlr.com) using flash.


any idea?

Maximum number of authorizations reached!
